About the Role
We are seeking a compassionate and bilingual Care Coordinator to support elderly patients enrolled in a U.S. Chronic Care Management (CCM) program.
In this role, you will build ongoing relationships with patients by conducting monthly check-in calls, reviewing medications, documenting patient interactions, answering questions, and helping ensure patients receive the support they need between physician visits.
This is a patient-focused role requiring empathy, professionalism, excellent communication skills, and accurate documentation. Experience in healthcare is required.
Key Responsibilities
Patient Care Coordination
Conduct scheduled outbound calls to elderly patients enrolled in Chronic Care Management (CCM)
Build trusting relationships through compassionate and professional communication
Answer patient questions and provide non-clinical education and support
Encourage patients to follow their physician's care plan
Escalate urgent medical concerns according to established protocols
Medication Review & Documentation
Review and update patient medication lists
Perform medication reconciliation during patient calls
Document patient interactions accurately and thoroughly for Medicare compliance
Record changes in medications, symptoms, and health concerns
Maintain complete and organized patient records
Care Coordination
Coordinate follow-up activities with healthcare providers when appropriate
Track monthly patient engagement requirements
Maintain consistent communication with assigned patients
Identify barriers that may prevent patients from following their treatment plans
What We're Looking For
Previous healthcare experience required
Experience in Chronic Care Management (CCM), care coordination, home health, medical assisting, nursing, or medical office operations preferred
Fluent English and Spanish
Clear spoken English with a neutral, professional phone presence
Strong customer service and communication skills
Compassionate, patient, and empathetic personality
Excellent documentation and organizational skills
Comfortable speaking with elderly patients in the United States
Strong computer skills and ability to learn healthcare software
Based in Santiago and available for full-time in-office work
Bonus Qualifications
Medical Assistant, Nursing, LPN, RN, or other healthcare certifications
Experience with Medicare programs
Experience with Electronic Medical Records (EMR/EHR)
Medical scribing experience
Knowledge of diabetes, hypertension, COPD, heart disease, or other chronic conditions
