Job Description
This is primarily an office-based role requiring frequent use of phone and computer systems. The Home Care Coordinator is responsible for managing and scheduling caregivers to ensure consistent, timely coverage for all assigned clients. This role is essential in maintaining continuity of care, handling last-minute staffing needs, and ensuring full compliance with agency policies and client requirements.
Key Responsibilities
Coordinate daily caregiver schedules to ensure all client cases are fully staffed (may require occasional after-hours or weekend availability).
Assign caregivers based on client needs, preferences, and qualifications.
Manage call-outs, no-shows, and last-minute schedule changes efficiently, while promptly notifying HR, Compliance, and the Director of Patient Services.
Communicate clearly with caregivers, clients, and internal teams regarding scheduling updates.
Maintain accurate scheduling records in systems such as HHAX or similar platforms.
Ensure compliance with authorized hours, care plans, and agency policies.
Proactively identify and resolve staffing gaps to prevent service disruptions.
Follow up on missed visits and document all actions taken (in HHAX and/or via email).
Support onboarding coordination by aligning caregiver availability with client assignments.
Collaborate with HR, Intake, Compliance, and Authorization teams to ensure smooth operations.

Work Schedule
The role operates within a scheduling window from 6:00 AM to 9:00 PM.
Shifts are rotational on a weekly basis within this time frame.
The position consists of a total of 40 working hours per week.
Flexibility is required to support operational needs, including occasional extended hours or weekend coverage when necessary.
